Ohio law requires any person who supplies gaming related equipment, goods, or services to a casino operator to obtain a gaming related vendor license. To complete the application to obtain such license, follow these instructions.
The following forms must be downloaded to your computer, completed in their entirety and submitted, along with any other needed documents or exhibits, to the Ohio Casino Control Commission through a secure transmission.

When you submit the Institutional Investor Certification please note the name of the entity whose securities are owned.
In order to securely transmit the completed documents, the below Request for Secure Transmittal of Gaming Related Vendor License Application must be completed. A non-refundable $10,000 license application fee is required in order to complete the application process. The fee must be paid by ACH Credit. Once the request is received, you will receive an email in 1-2 business days from the Commission providing instructions for securely submitting your application materials and instructions on how to pay the $10,000 application fee through the ACH Credit process.
